how can a muslim know for sure they will go to heaven, or paradise, whatever you call it? If you can know for sure, then can you know it will be forever, or can something you do in your lifetime disqualify you after you have done the requirements to get into heaven?

the messengers that came to us gave us a simple message worship allah alone with no partners and you will be gurenteed paradise we belive that every person who belives in god will enter heaven even other religions but not idol worshippers or athiest but the people who didnt worship God the correct way and disobeyed or didnt follow some of his commands will be sent to hell or be punished in the grave before entering heaven and even when you do enter they will have black faces and everyone will know they were once in hell as for those who follow the religion correctly will go straight to heaven but we also have a book of deeds and a scale if the scale is heavier at good deeds you go to paradie if its heavier with bad deeds you go to hell untill you have been punished for the sins you commited only certain things can send you straight to hell like adultary, killing somone, shirk, or worshipping somone or somthing other than god, but some deeds can be payed off in hell and then you may enter paradise when you have been punished for you deeds but there are always things that allah forgives before you die, allah forgives all the sins of a martyr and if you truley repent you bad deeds will be forgived our prophet said everymuslim is gurenteed aradise and on that somone asked even the adulteres and the hypocrites and he said yes even the adulteres and the hypocrites

We won't know whether we have got into heaven/hell until the day of judgement. and if you commit shirk (idolatry or associating beings or things with Allah) all your good deeds will be erased and you will go to hell, that's the only one i can think of at the moment :)

unless you repent before you die that is, then your shirk will be forgiven. sorry forgot to add this

okay, so where in your Quran does Allah promise that you can be saved? and what about the other question...can you be " unsaved" in the Muslim religion?

this is one i can think of on the top of my head from the quran:

Their reward with Allah will be gardens of perpetual residence beneath which rivers flow, wherein they will abide forever, Allah being pleased with them and they with Him. That is for whoever has feared his Lord.98:8
